CHKD's Sports Medicine Physical Therapy is excited to offer a sports screening to figure skating athletes at our CHKD Oakbrooke Sports Medicine Gym. This screening is for athletes under the age of 21 who skate at Basic Level 5 or higher. 

When completing your registration, please only select ONE session time for each individual athlete. 

Please bring sneakers with you and arrive 15 minutes prior to your session time to complete paperwork.
